Abstract

A 34 year old male was admitted with progressive general malaise, headache ana
petechiae on both extrimities.
Physical examination revealed severe pale conjunctivae.
Hematologic study showed pancytopenia with leukoerythroblastosis and apparant
dysplasia of trilineage blood cells.
Bone marrow was hypercellular and especially erythroid and megakaryocytic
hyperplasia were evident. On the iron staining, sideroblastic count was increased.
2 years later, hematologic findings were exacervated and bone marrow, aspirated
with difficulty, was "dry-tapped". The marrow section showed marked myelofibrosis
with increased reticulin fibers on reticulin stain.
Cytogenetic study showed normal karyotype, but on the hemopoietic cell culture of
peripheral blood, cultured cells were abnormal at the aspects of maturation and
differention.
